
Carer respite
We believe that to successfully care for our clients, we also must care for those around them! We appreciate having to open a home to health care workers can be difficult, especially at first. We recognise that being a carer for someone is a big change and at times can be tiring. At Sphere Specialist Healthcare, we believe that family members, carers and friends are all an integral part of the client’s care and as such we are all a part of the same team. Our support workers are all happy to chat, listen and support family members and carers at any time. We realise the importance of family and friends who provide care staying well, both physically and mentally, maintaining a good quality of life and having the ability to meet responsibilities that aren’t related to care giving. Should you need periods of respite, be this a couple of hours once a week, overnight care to get a good night’s sleep or longer periods several times a week we can help. This can be arranged at short notice in emergency situations or booked in advance.
Enablement
At Sphere Specialist Healthcare, we look at the bigger picture. Whilst we understand how important it is to ensure healthcare needs are met, we recognize there is so much more to a person than their healthcare needs! Our highly skilled staff can support clients to:
- Enjoy activities within their local community,
- Explore the beautiful landscapes in the area,
- Carry out day to day tasks, such as shopping, picking up medication, going to the bank, walking pets and attending hospital appointments or paying bills,
- Visit family and friends.


Shopping
Need a hand with the weekly shop? Or a few bits day to day?
We can help! Our support workers will help collect any shopping needed whether this be a regular thing or a one off due to illness or family not being available to help. We can support clients to compile a meal plan and shopping lists and carry out shopping on their behalf. We will then return the shopping to the client and support with storing it appropriately. We can also support with online shopping and arrange delivery for when the support worker is at the home to help put the shopping away.
Housework/Domestic
It is important for our psychological wellbeing to be in an environment where we feel comfortable. It is also important (now more than ever) to be in an environment which is regularly cleaned to stop the spread of bacteria and germs. We can help with housework or domestic tasks to ensure that clients feel comfortable and safe in their environment. We will include this in the plan of care to ensure that all of our support workers are aware what tasks are to be carried out, if there are any tasks the client wants to be involved in but needs support to accomplish and how the client likes tasks to be carried out.


Personal hygiene
At Sphere Specialist Healthcare we would like all our clients to enjoy high self-esteem and to feel good about themselves. Evidence shows that to enjoy good self esteem we need pride, self-confidence, self-respect, a sense of identity, healthy humility, and appreciation for ourselves. We aim to support our clients to achieve all of the above. We know that for our clients, having someone support them with their personal care needs can be daunting and a new experience. We work with our support workers to ensure this is carried out with respect, privacy and dignity at all times and take great care to acknowledge the sensitive nature of this aspect of care. We recognise that everyone has different routines, standards, and ways in which they carry out their personal hygiene and will work with clients and their families to carefully develop an individual and bespoke plan of care for how they would like personal care to be delivered. We will work to promote individuality, choice and independence at all times to ensure that the client is comfortable, and our professionals will perform risk assessments with the client to ensure this is safe, for example entering and exiting the shower. We can provide services including washing and dressing, bathing, showering, shaving, oral hygiene and continence care at any time of day to suit the individual’s preference and need.
Medication Administration/Monitoring
Ensuring that medications are given safely and accurately is a cornerstone of safe care. We recognise that medication is most effective and maintains therapeutic effect when given exactly as prescribed by the prescriber and develop individual care plans around this. Our support workers all go through rigorous and regular training in medication administration and have competence assessments performed by a Sphere Specialist Healthcare Registered Nurse. This ensures they have full understanding of the importance of their action and deliver this care in a safe and knowledgeable manner.


Prescription pick up
At Sphere Specialist Healthcare we recognise the significance of collecting prescriptions on time and having these in the home, so they are available to the client when needed. Delays in this can result in a break in the effectiveness of the medication and the client having an increase in symptoms of their illness. We can collect prescriptions and take these to the client, should this be a one-off emergency, weekly or monthly requirement.
Monitoring of Health Conditions/Ensuring Wellbeing/Telecare
At Sphere Specialist Healthcare we recognise the importance of ensuring symptoms of long-term health conditions are kept to a minimum. This ensures that the client enjoys optimum health and a high quality of life.
We can support with the following:
- Diabetes Management including Blood Glucose Monitoring
- Blood Pressure Monitoring
- Oxygen Saturations Monitoring
- Wound Care
- COPD Management/Initiation of Rescue Medication if Prescribed.


Meal Preparation/Feeding
At Sphere Specialist Healthcare, we recognise the importance of good nutrition and hydration in both physical and mental health:
We recognise the importance of adhering to dietary advice and ensuring supplements are taken when prescribed. We work collaboratively with other professionals to ensure we adhere to all guidance in this area. Our team of nurses work with support staff to ensure they are aware of the impact good nutrition and hydration has on the body, including where health conditions require special diets and the importance of leaving snacks and beverages between visits. We can schedule visits to prepare meals or to support or supervise clients to prepare their own meals. This could be once or several times a day and our team will work with clients to carefully plan requirements and preferences in this area to ensure our clients enjoy all the benefits good nutrition and hydration can give.
Specialist Healthcare Tasks
At Sphere Specialist Healthcare we recognise the desire to be cared for at home instead of having to go into hospital. The shift in social care is for more and more tasks to be carried out at home by care and support staff or family members than ever before. We understand that although this is preferential it can also cause a little apprehension. We have a team of professionals including nurses from various specialities and an occupational therapist who all have much experience of delivering complex care in the community. Their goal is to ensure both the client and their family are comfortable with and confident in the care delivered. We provide training to extremely high standards to ensure that our support workers have not just the skill in carrying out tasks, but the knowledge that underpins them to ensure this care is always delivered safely. Our team of professionals will develop a bespoke care plan around the clients needs and preferences in relation to these specialised tasks and work alongside support staff for periods of time until everyone is comfortable for the support worker to work independently. Our professionals are always on hand to answer any questions that the client, family or our support workers may have to ensure everyone is comfortable giving and receiving care.


Reablement
Sometimes, care or support is needed only for a short time while you recover from illness or injury. We can develop a support plan with you to reflect this incorporating any of our other services as required, such as personal care, shopping, prescription pick up or enablement.We can also work with you to set goals and break these down to help achieve them fully. We can also support you with any rehabilitation you may need working with your existing healthcare team for guidance and planning.
Community Hub
Our community hubs deliver a safe and enjoyable space for people with a Learning Disability, Autism and/or Mental Health conditions. The aim of the hubs is to support individuals to set, progress towards and meet their own goals and targets, whatever these may be. This may be moving into volunteering or employment, developing skills for activities of daily living, or engaging regularly in activities which are enjoyable and offer holistic development or progression for the individual.
We support individuals to contribute to society in a meaningful way and enjoy the benefits of being a part of the community in which they live, moving away from the traditional day centre model to make the service a more individualised and meaningful experience.
Due to the experienced team of nurses we have available, we are able to cater for those who also have complex physical health needs.
