Sphere Specialist Healthcare Ltd was established in 2021 by Carly Fortune, supported by a range of health professionals who collectively have a wealth of knowledge from both the health and social care fields.
Our primary emphasis is to support and enable individuals to lead a fulfilling life in the way that they choose, supporting people to live independently for as long as possible.
Aim
We provide a range of specialist high quality and bespoke care services to clients who need support to live independently. We aim to ensure our clients needs are met holistically, to not only support clients to live at home for as long as possible, but also to have an active role within the community and thus and improved level of meaningfulness and greater self-esteem.
We provide services to a wide range of clients including:
- Older people
- People living with a physical disability
- Those recovering or readapting following a hospital stay
- People diagnosed with long term conditions
- People living with a learning disability/autism.
We can deliver our services within the client’s home itself or various locations within the community in line with the clients individual and bespoke care plan. All services are fully assessed for risk by qualified nurses, occupational therapists or other specialists deemed appropriate for that individual activity or location.
We pride ourself on our attention to the detail enabling us to provide a safe, caring and unique service to all.

We understand that care is a journey, and we want to support you on this journey. We recognise that no two journeys are the same and this is also true in providing care for someone. Our service is bespoke and individual to each client we support. We take pride in reviewing and adapting the service we provide to meet the ever-changing needs and preferences of our clients as individuals. We recognise the importance of supporting staff through their career journey and promote an environment of continuous learning for all staff. We believe that there is always more to learn and work with staff throughout their journey to develop any areas of specialist interest that they may have or any area where they feel they wish to improve. We tailor training plans to the individual and their goals and review these regularly and adapt as needed or requested.
Objectives
- To provide high quality, person-centred care to enable people to manage their health conditions with a level of support which is appropriate to them as an individual and allows for personal growth and independence.
- To design and deliver bespoke care plans, working in partnership with the client, relatives and other health professionals involved in care delivery.
- To support clients to set their own goals and plan for how they can be achieved and what support will be needed for this to occur.
- To support clients with a view which is balanced in terms of safeguarding them from harm whilst acknowledging their right to take their own risks with appropriate information, planning and support.
- To ensure staff have up to date knowledge of the care and interventions they are delivering and are suitably skilled to carry out all tasks required for the clients they visit.
- To ensure the service is flexible and attentive to the detail of care provided, including when needs, condition or preference may change.
- To deliver care which promotes equality and is inclusive, non-judgemental and without prejudice to all, especially in relation to age, gender, race/ethnic origin, culture, religion, sexuality, sexual orientation, political affiliation, parenthood, marital status and disabilities or impairments.
