Health Monitoring, Screening & Long-Term Condition Support
At Sphere Specialist Healthcare, we understand that small changes in a person’s health or wellbeing can be an early indication that something isn’t quite right. Our trained team can carry out a range of health observations, assessments and screening within the client’s own home, helping to recognise deterioration earlier and ensure concerns are acted upon appropriately.
Our support can be tailored to individual needs and incorporated into the person’s care plan. This may include:
Blood pressure, pulse, temperature and respiratory rate monitoring
Oxygen saturation monitoring
Blood glucose monitoring and diabetes support
Weight and general health monitoring
UTI screening and assessment
Falls assessment and post-fall monitoring
Head injury assessment and monitoring
Wound and skin monitoring
Depression and wellbeing screening
COPD and other long-term condition monitoring
Other appropriate health screening and assessments according to individual need

WHZAN Blue Box Technology
We can enhance this support through WHZAN Blue Box technology, enabling our trained team to undertake and digitally record a range of health observations and assessments during visits.
The system provides a structured digital record of observations, helping us monitor changes and trends over time rather than viewing each observation in isolation. Where the appropriate observations are recorded, the technology can also support calculation of a NEWS2 (National Early Warning Score 2).
NEWS2 is a nationally recognised clinical scoring system widely used across the NHS to identify and respond to deterioration in a person’s physical health. Using this recognised framework means that, when concerns need to be escalated to NHS or other healthcare professionals, the significance of the score and the observations contributing to it can be clearly understood, supporting effective communication and timely clinical decision-making.
This combination of technology, consistent digital record keeping, clinical observations, recognised assessment tools and trained staff provides an additional layer of reassurance.
Where monitoring, screening or a NEWS2 score identifies a potential concern, our team can escalate this appropriately to the relevant healthcare professional or emergency service, providing clear information about the observations recorded, changes identified and the individual’s current condition.
Our aim is not simply to respond when somebody becomes unwell, but to recognise potential concerns earlier, identify changes from an individual’s usual health and support timely intervention wherever possible.
Proactive monitoring. Better information. Earlier recognition. Helping people remain safely supported at home.
